CDH Rehabilitation Specialists collaborate closely with patients, family members and other caregivers to assure a continuity of care from the medical center to the home environment (or to an intermediate care facility, when appropriate).
An individualized home program is an important step in rehabilitation. Our therapists educate patients, family members and caregivers about home care and how to reinforce rehabilitation at home. We also incorporate the family/caregiver’s goals into our overall rehabilitation plan.

Central DuPage Hospital (CDH) Rehabilitation Services address a wide range of needs: restoring strength after surgery, learning to speak again after stroke, overcoming congenital limitations and compensating for functional loss caused by disease, injury or treatment. Each patient’s individualized therapy program is based on comprehensive evaluation, planning and personalized goals. Rehabilitation services include: Physical therapy, Occupational therapy, Speech therapy, Aquatic therapy, Chronic pain syndrome therapies, Cognitive rehabilitation, Cleft lip and palate clinic (assistance with swallowing and speech), Ergonomic assessment, Hand therapy, Language therapy, including articulation; Lymphedema rehabilitation; Neurologic rehabilitation; Orthopedic rehabilitation, including after hip or knee replacement surgery; Orthotics fabrication; Pediatric rehabilitation; Pediatric developmental followup; Pelvic pain/incontinence therapy; Pregnancy-related treatment; Rehabilitation after treatment for head and neck cancer; Screening clinics, including speech development, fine motor skills, gross motor skills, sensory processing and others; Spine-related injury rehabilitation; Sensory integration and processing; Sports-related injury rehabilitation; Swallowing disorders assessment and treatment, including videofluoroscopic evaluation and VitalStim therapy; Tracheotomy rehabilitation; Ventilator-dependency rehabilitation; Vestibular and balance disorders treatment; Voice assessment and treatment; Wound care .
