Short profile:
Continuous Care Center is licensed as a general acute care hospital. We are a prime example of the new type of health care facility that is dedicated to providing quality acute care to medically complex, chronically or acutely ill patients who often require state-of-the art medical technology.
Our patients are usually those in transition from another medical environment such as intensive care units, and general acute care. They do not fit the usual critical pathway and strict DRG guidelines of the traditional hospital.
Detailed description:
Our hospital services complement the care provided by traditional hospitals, skilled nursing units and rehabilitation hospitals. Our mission is to provide high-tech resources with individualized care for the "careful to recover" patients, in a non-profit environment.
Our patients are usually those in transition from another medical environment such as intensive care units, and general acute care
These are patients who have prior medical problems that complicate and lengthen their full recovery period. Our typical patients need continued medical management on a daily basis and nursing care complemented by various therapies that focus on moving patients from an acute setting toward maximum independence.
There are several key differences between a traditional hospital and the acute long-term hospital like Continuous Care Center. CCC traditionally has higher nursing, respiratory, and rehabilitation staff-to-patient ratios. The same interdisciplinary team treats our patients throughout their stay at CCC.
