CONTINUOUS CARE CENTERS

Our newly relocated 18 private bed facility is located on the 7th floor of OSU Medical Center hospital. Our medical teams provide adult patients, who require longer transition of treatment, with a variety of rehabilitative services designed toward maximum independence.

Our hospital services complement the care provided by traditional hospitals, skilled nursing units and rehabilitation hospitals. Our mission is to provide high-tech resources with individualized care for the "careful to recover" patients, in a non-profit environment.

At Continuous Care Centers, we have relied on a highly involved team treatment approach from the very beginning. A multidisciplinary team of physicians, nurses, therapists, social workers, and counselors all work together to provide a full range of acute and rehabilitative services.

Comprehensive social services, internal case management, counseling, and a highly structured cognitive remediation program are provided. Neuropsychological evaluation, physical medicine, and rehabilitation management programs are also available as needed.

We consider family involvement in the treatment program to be a vital component to our team approach. Our comprehensive plan includes education programs to teach families how to play an integral role in the patient’s rehabilitation.
