CORNELL SCOTT-HILL HEALTH CENTER - GRANT STREET PARTNERSHIP

GSP provides partial hospital, intensive outpatient and outpatient programs for men and women with substance abuse problems, mental health, and transitional housing for homeless males. Primary medical care is available for program members.

Day Programs: Monday - Friday from 9 am - 1:30 pm; Evening Programs: Monday - Thursday from 6 pm - 9 pm.

Clients are provided with three substantial meals a day at Grant Street Partnership. Transportation is provided for both clients residing in the shelter component and for those attending treatment on an outpatient basis. Grant Street Partnership provides three levels of care for all clients.

Partial Hospitalization Program (PHP): 2.5 level of care; 9 am-1:30 pm, Monday through Friday.

Intensive Outpatient Treatment (IOP): 2.1 level of care; 6 pm-9 pm, Monday through Thursday evenings or 9 am-12 pm, Monday through Thursday.

Outpatient or Aftercare Programs: 1.1 level of care; up to two groups and one individual session per week.

The components of the program include:

Therapeutic Milieu, Individual Therapy, Group Therapy, Twelve-Step Education/AA and NA Meetings, Case Management/Housing Assistance, Life Skills Training, Primary Health Care and Referrals, Psychiatric Evaluation and Medication Management, Social Support Development, Discharge Planning, Job Readiness, Entitlements/Basic Needs, HIV Counseling.
